SUBMITTING CLAIMS TO YOUR INSURER

Co-payments and unmet deductibles are due on or before your date of service.  We will submit your bill directly to your insurance carrier.  A bill will be sent to your secondary insurance carrier upon receipt of payment or denial from the primary insurer.  If you have no secondary insurance, a bill will be sent to you for any balance after receipt of payment or denial from your carrier.  We must make a copy of each insurance card at the time of registration.

SELF PAY AND COSMETIC SURGERY

You will be contacted prior to your surgery with an estimated procedure cost for you surgery.  Projected self-pay amounts must be paid at the time of service. If you are unable to pay the entire amount estimated you will be asked to complete a promissory note with a limited number of installments.
